Aetna Infertility Benefits

Follow the steps below to get started, and view Aetna’s Infertility Treatment FAQs for more information.

  1. Contact the REI Health Guide at 1-800-451-2967 to confirm your eligibility.
  2. Work with your physician to obtain pre-certification before utilizing services.
    • Infertility is covered in-network only. Log in to your member website, complete the registration form and submit it to the National Infertility Unit (via fax at 1-860-607-7476).
    • Infertility providers are responsible for treatment authorizations.
    • The REI Health Guide will contact the Express Scripts’ Client Service Team with the pre-certification approval.
  3. Coordinate your prescription with Express Scripts. To learn more, review the Freedom Fertility Benefit.

Aetna Maternity Program

Aetna’s Enhanced Maternity Program can help. The program, staffed by nurses with maternity experience, provides:

  • Educational materials (available in English and Spanish) on prenatal care, labor and delivery, and newborn care
  • A pregnancy survey to help determine whether a risk for certain complications exists
  • A pre-term labor program to support high-risk women
  • Access to specially trained nurses for high-risk mothers-to-be
  • One-on-one counseling to help pregnant women quit smoking

Joining is easy and there is no extra cost to you. Sign up at Aetna.com > Health and Wellness or call 1-800-272-3531 (TTY: 711) and complete a pregnancy survey to help Aetna get to know you better. The survey contains a few questions about medical conditions and behaviors that can influence your pregnancy. Your answers to the survey qusetions are proviate and don’t affect your eligiblity for health coveage.

Adoption Assistance Program

Are you considering adoption? REI Benefits Plan-eligible part-time and full-time employees who have worked for REI for 12 consecutive months are eligible for up to $7,500 in reimbursement per adoption to help with expenses directly related to the adoption process.

To prepare yourself for adoption, review this resource, which includes:

  • A checklist that walks you through the REI benefit programs available to you as you go through this process;
  • The Adoption Assistance Reimbursement Form you can submit to be reimbursed for eligible expenses; and
  • The Qualified Life Events Form, which you will need to complete to enroll your child in REI medical coverage.

Once the adoption is final, submit the Adoption Assistance Reimbursement Form and copies of your related receipts to: “HR, Attn: Adoption Assistance.” Your request will be processed within one to three weeks. Adoption Assistance Reimbursements are subject to FICA and FUTA (payroll) taxes, but not personal income tax.
