What are My Responsibilities During a Leave?

While you are out on a leave of absence, taking care of yourself or your family is a top priority. To ensure your leave runs smoothly, please make sure you are aware of and complete your responsibilities while on leave.

1. Stay in touch with your manager.

While on leave, it’s your responsibility to stay in touch with your direct supervisor and Lincoln Financial. As you approach your expected return-to-work date, notify them if you need to remain on leave past the original return-to-work date.

If you need to extend your leave, share your expected return-to-work date and notify your supervior and Lincoln Financial of any accommodations that you’ll need when you return to work.

2. Submit documents on time.

It is important that you submit any documents required of you or your treating provider. Delays or failure to provide documentation or medical records that are accurate, signed or readable may mean your leave and claim approval is delayed or denied, which will impact your pay. It is your responsibility to follow up with your physician to ensure medical documents are submitted.

Depending on the reason for your leave and if you’re eligible for disability pay, you will need to complete forms provided by Lincoln Financial. You can view the leave letters and attachments that have been sent to you on My Lincoln Portal under the View Status > Correspondence tab. You can also view blank forms on My Lincoln Portal under Forms & Documents.

  • Certification of Health Care Provider (employee’s own illness or care of a family member): This needs to be completed and returned to Lincoln Financial within 15 days.
    • Section 1: To be completed by you.
    • Section 2: To be completed by your qualified health care provider.
       

      This form is needed to approve your leave of absence. You should always follow up with your doctor to make sure the form is completed and returned by the due date.

  • Declaration of Birth: For the birth parent or parent taking bonding leave, you may have the Declaration of Birth form completed by the attending physician, as an alternative to providing the birth certificate.
  • Authorization to Release Medical Information: To help expedite the release of medical records, provide your physician a copy of the release at your next visit. This form gives your doctor permission to release your medical information to Lincoln Financial.

3. Read and understand all the information and instructions.

Read all the information and/or instructions provided and know what you have been approved for. (This may be different than what your doctor determines.) Lincoln will notify you via mail, email and/or phone during your leave. Make sure you read any communication from Lincoln and respond in a timely manner, and complete any additional documents needed for your leave and disability pay. If approved for an intermittent leave of absence, you must report your intermittent absence within 5 calendar days, by 11:59 PM EST, of your absence. You can report your absence 24 hours a day, 7 days a week through Lincoln Financial online through mylincolnportal.com or via phone 1-888-970-2457, Option 4.
